Required Forms

All campers are required to complete the UH Medical Information Form and the UH Indemnification and Release Form. These will be sent to your email (check your spam folder) and can also be downloaded from our website. Catchers for pitching sessions will also need to fill these forms out unless they are registered for another session that same day and have already completed their forms. Please email scanned copies or pictures of signed forms to chrissyschoonmakersoftball@gmail.com prior to the clinic or bring printed copies with you when you check in. These forms are required every time even if you have attended a previous clinic with us.

*The UH Indemnification and Release Form will not allow you to sign it on a computer or phone and will need to be printed to sign. The UH Medical Information form is able to be filled out on a computer or phone if you would like to do so.
